Forms

All forms must be filled out before the 1st day of class.

The first step in the program is completing an assessment Assessment are $125. Please be aware we do not take insurance so it will be your total responsibility to pay for the assessment. By completing the assessment it will help us determine if the class is appropriate for you to take.

To download all forms please click on the arrow on the right hand corner. Once you download you can then began to type your information on the form. Please send the forms back in an attachment by email. In order to save the form on your computer after you completed it ;click on the right hand side of the mouse. To name the form you want to save as and name the forms by its name. Go to your email and add the attachment and send it to angerbuster123@trainingacademy980.com
